feedback from the primary?

Hi All,
I am writing to make sure we have as much feedback as possible on any problems with voting during the August primary in Colorado.  Such information might still be helpful for the lawsuit currently underway (which sues the Colorado Secretary of State for unlawfully certifying some voting machines.)
First, did you personally notice problems as a voter, poll watcher, election judge, member of a canvass board, etc.?
ALSO, if you are in a position to ask others in a related organization whether THEY had problems, this would be very much appreciated.
Please try to note the following:
1.  Name and contact information of person observing the problem.
2.  The precinct  (or vote center) number or location (or person's zip code, if they can't remember information on the voting place).
3.  The county
4.  The voting machine used
5.  Type of machine problem:
     a. printer jams
     b. booting problems
     c. discrepancy between the ballot cast on a touchscreen and the paper ballot to be verified by the voter
     d. vote switching: the machine registers a different candidate than the one voted for
     e. problems with disabled voting: with audio, etc.
     f. chain of custody: how voting machines and memory records were transported and accounted for before, during, and after the election
    g. machines unsecured, unattended in public places
    h. machines on when polls are closed, or off when polls are open
     i. any other voting problems.

Information will be most useful if it reaches us by this coming SATURDAY, AUGUST 26th.  Please call Claudia Kuhns of the Be the Change Public Integrity Project at 303-744-3407, or her cell: 303-349-7583.
Thanks very much.
